A decline in mobility following a hospital stay or due to a long term health condition, can feel overwhelming. One week youโre managing fine at home, the next you or a loved one need help with the basics: getting washed, getting dressed, moving safely around the house.
At Warwickshire Home Physio, we offer a focused, intensive rehabilitation programme designed to help you regain the confidence and physical ability to care for yourself at home. This can help reduce or remove the need for ongoing carer visits altogether and get you back on track with daily life.
Who This is for
This service is for anyone who:
- Has recently been discharged from hospital and is finding day-to-day tasks harder than before
- Is currently receiving, or has just finished, a period of NHS reablement support but still lacks confidence doing things independently.
- Has carer visits in place for washing, dressing, or moving around the home, and would support to increase their independence.
- Has experienced a sudden decline in mobility and wants focused, hands-on support to rebuild strength, balance, and confidence.
How Does Intensive Rehabilitation Work?
We start with a thorough assessment. One of our Physiotherapists visits you at home to understand your current ability – how you move, your strength and balance, your home environment, and exactly which everyday tasks feel difficult or unsafe.
If youโre ready, we deliver intensive rehabilitation, this combines hands-on support with essential daily tasks – like washing and dressing – alongside targeted exercise sessions designed to build the specific strength, balance, and confidence needed to do those tasks independently.
Everything we do is built around the specific tasks that matter to getting you back to independence. Your goals, your home, your daily routine, not just a generic exercise plan.
